We are looking for people aged 18 and over. We would like to hear from you if:

– You are living with a life-limiting illness and have experience of being cared for by a Virtual Ward or Hospital at Home Service

or

– You have experience of looking after a loved one living with a life-limiting illness who received or is receiving care from a Virtual Ward or Hospital at Home Service
