If you have been a patient at Queens Medical Centre or Nottingham City hospital in the last 12 months and have been moved from one ward to another during your stay we want to hear about your experience.
We want to improve the care of patients who are transferred and help healthcare staff spot when a patient is getting more unwell and needs help quickly.
