We are planning research to help GP surgeries better identify people living with multiple long-term health conditions who may be more likely to be admitted to hospital. We would like to hear from people with two or more long-term conditions, and family members or carers, about their experiences of healthcare and get their views on our research plans.

The University of Birmingham is known for its teaching, healthcare research, and work to improve health and social care services.
This work will help inform a funding application for research aiming to improve how we identify people with multiple long-term conditions who may be at higher risk of serious health problems or hospital admission.
Within the research team are two researchers who also work part time as NHS GPs.
